High School Faith Formation

Service Hours

A very important part of our formation in the Catholic Faith is doing service to those in need. We encourage you to seek out opportunities to serve whenever you can! Remember it is not about “getting hours” but it is about showing love by serving others. At the same time, for the purposes of our program, we need to keep track. Please use this form to log your service hours and submit them to your teacher.

Special Education Individual Faith Formation Plan

In order to best serve students who may have additional learning needs, Immaculate Conception is developing Individual Faith Formation Plans. These documents will serve as a guide to help students enhance their Catholic faith and pair them with opportunities to share their gifts in our community. Kathy Green, a special education teacher, is joining the Faith Formation team to spearhead these efforts.

Families with questions about Individual Faith Formation Plans should contact Gail Capozzi @

  • Registration form *** Legible Email address is essential*** with payment
  • Copy of Student’s Baptismal Certificate (Or indicate date of Baptism at IC)
  • Volunteer sheet for Parents

***Registration will not be complete without Baptismal information***

We will meet monthly on Sunday mornings from 10:15 a.m. – 12:30 p.m. Please note that students should have regularly attended Faith Formation classes for 7th and 8th grade. If your child did not regularly attend those classes, there will be a make-up requirement prior to the start of Confirmation classes. If your child needs to make up some of that work, you will have received a letter in June.
